Baptism
Form
In Baptism the form are the words "I Baptize you in the Name of the Father and of the Son and of the Holy Spirit." These words are spoken by the one baptizing in order for it to be a valid baptism.
Matter
The primary matters of Baptism are water and someone to baptize. It doesn't need to be holy water.
Secondary matters are:
a priest or deacon because in an emergency anyone can baptize someone
oil
white garment
Minister
The minister of Baptism is usually a priest or deacon but anyone can baptize someone in an emergency
Rite
The minister pours water over the forehead of the receiver three times saying "I Baptize you in the Name of the Father and of the Son and of the Holy Spirit." as the rite of baptism.
Effect
The recipient enters the church and is wiped away of original sin
